You will be required to apply in person in any of the following conditions are true: you are applying for your first passport; your previous passport was issued when you were under age 16; your previous passport was lost, stolen, or damaged; your previous passport was issued over fifteen years ago; or your name has changed since your passport was issued and you are unable to legally document the name change.

The first step is to obtain and fill out a Form DS-11 (Application for a U.S. Passport). In order to submit this form, you must apply in person at an Acceptance Facility or Passport Agency. You also must supply any documentation specified in the document. Make sure you do not sign the application until you are instructed to do so by the Acceptance Agent, or it will be void. Additionally, you will be required to provide your Social Security Number.

When applying in person, proof of citizenship must be submitted with Form DS-11. These documents will be returned to you either with your passport or in a separate mailing. This proof can include one of the following: previously issued passport; certified birth certificate issued by the city, county, or state; Consular Report of Birth Abroad or Certification of Birth; Naturalization Certificate, or Certificate of Citizenship.

When applying in person, you must also present acceptable identification, including the following: previously issued U.S. passport; Naturalization Certificate; valid driver’s license; current government ID (city, state, or federal); or current military ID (military and dependents). A photocopy of the ID must be taken, including both sides of the document.

Additionally, you must provide two passport photos that are identical and in color. The fees will vary from $45 to $120, depending on what you are applying for.
